  Practice Information Sheet is made available to all new patients. If patients are unable to read or understand the information sheet assisting translation service is provided.

Reminder Services
City West Medical Centre has a reminder system in place through our medical program.
Federal Reminder Systems
There are reminders from the Government sent to you to remind you of immunisations, pap smears and Breast Screening.
Communication with patients via electronic means (Fax or Email)
Communication with patients via electronic means (Fax or Email) is conducted with appropriate regards to the privacy laws relating to health information and confidentiality of the patients health information. Patients can request our written policy on receiving and returning electronic communication. All emails are first seen by the Practice Manager and then allocated to the relevant doctor. All faxes and emails constitute an official record and will be part of the patient's medical record.
